On this World Kidney Day, we want to raise awareness of the importance of early diagnosis of Diabetic Kidney Disease (DKD) through a DKD Screening Program in Community Pharmacies (~100 pharmacies) in Portugal.
The activity has the following objectives:
1. Provide technical-scientific training on kidney disease to pharmacists and pharmacy technicians
2. Implement DKD awareness campaigns in community pharmacies (online and offline) directed to diabetic patients.
3. Identify undiagnosed diabetic individuals at risk for DKD, at the retail pharmacy level, through a point-of-care test to analyze the albumin/creatinine ratio.
4. Refer diabetic patients at risk to a formal screening at the primary healthcare.
5. Follow-up for screening results and clinical practice implemented for patient treatment.
If you are living with diabetes, after diagnosis, you should be assessed once a year to identify possible kidney problems. If you have type 2 diabetes, this call is for you! Don’t turn your back on your kidneys. Visit our pharmacy and get screened for Diabetic Kidney Disease.
