Diabetic Kidney Disease (DKD) Screening Intervention in Community Pharmacies

In Portugal, 14.1% of the adult population is diabetic. For these patients, tight control of their blood glucose level is crucial to decrease the risk of associated complications and to improve their quality of life. Around one in three people with the disease may develop kidney complications throughout their lives.

On this World Kidney Day, we want to raise awareness of the importance of early diagnosis of Diabetic Kidney Disease (DKD) through a DKD Screening Program in Community Pharmacies (~100 pharmacies) in Portugal.

The activity has the following objectives:

1. Provide technical-scientific training on kidney disease to pharmacists and pharmacy technicians    
2. Implement DKD awareness campaigns in community pharmacies (online and offline) directed to diabetic patients.
3. Identify undiagnosed diabetic individuals at risk for DKD, at the retail pharmacy level, through a point-of-care test to analyze the albumin/creatinine ratio.
4. Refer diabetic patients at risk to a formal screening at the primary healthcare.
5. Follow-up for screening results and clinical practice implemented for patient treatment.
